Services Built Around Each Person
Person-Centered Care, Every Step of the Way
Every individual we serve has unique strengths, goals, and needs. Our team works closely with each person — and their family — to create a care plan that reflects who they are, not just what they require.
As a licensed APD provider, all of our services are funded through Florida's Medicaid Waiver program. We guide families through the enrollment process and coordinate with APD support coordinators to ensure seamless, uninterrupted care.

Understanding APD Funding
Florida's Agency for Persons with Disabilities (APD) administers the Home and Community-Based Services (HCBS) Medicaid Waiver, which funds the services Heritage Care Home provides. Eligible individuals can access high-quality care at little or no out-of-pocket cost.
To receive APD-funded services, individuals must be enrolled with APD and have an active Medicaid Waiver. Our team is experienced in helping families navigate the enrollment process, understand their options, and connect with an APD support coordinator.
APD Enrollment
The individual must be enrolled with Florida's Agency for Persons with Disabilities and meet eligibility criteria.
Medicaid Waiver
An active Home and Community-Based Services (HCBS) Medicaid Waiver is required to access funded services.
Support Coordinator
An APD support coordinator helps develop the individual's support plan and authorizes services.
Begin Services
Once authorized, Heritage Care Home begins delivering personalized, person-centered care.
